Phacoemulsification and Intraocular Lens Implantation using the Capsular Tension Ring

In the cases of the surgical management of the cataract associated with extensive zonular weakness or dialysis, many surgeons prefer cataract extraction with implantation of sutured posterior chamber intraocular lens[PC-IOL]. However, sutured fixation of a PC-IOL significantly increases surgery time, long-term instability, and axial tilt of the IOL may occur postoperatively. The capsular tension ring may provide a safe means tomanage zonular dialysis during phacoemulsification through small incision, and good visual outcome can be obtained by allowing the intraocular lens easily placed into the bag. We reviewed the clinical records of 12 cataract surgery cases with loose or broken zonules managed with capsular tension ring. Postoperative visual acuity improved in all cases, and better than 0.5 in nine eyes[75%]. Postoperative complication included posterior capsule rupture[2 cases], vitreous strand incarceration[2 cases], increased intraocular pressure[1 case], but without decentration of PC-IOL. In conclusion, in the eyes with extensive zonular dialysis or lens subluxation, the capsular tension ring may provide significant advantages by enhanced intraoperative safety, earlier recovery through small incision phacoemulsification and long-term stability of PC-IOL.
