Lipogranuloma of Eyelids after Paranasal Sinus Surgery

Abstract

We describe a 33-year-old man who complained of chronic swelling of the left eyelid developed after frontal sinus surgery for frontal sinusitis 6 years ago. Firm, movable,and nontender masses were palpated measuring 2 x1 cm in the left lower lid and 0.5 x0.8 cm in the left upper lid. T1-weighted magnetic resonance imaging showed multiple small high-signal structures changed to signal-void structures with fat suppression. We removed the mass from the left lower lid. Histologic examination showed features characteristic of a lipogranuloma. They consisted of lymphocytes, epithelioid cells, foam cells, and foreign body giant cells surrounding multiple irregular vacuoles. We report one case of a lipogranuloma as an unusual complication of sinus surgery possibly due to the intraoperative nasal packing with petroleum-jelly based ointments.
