Clinical Results of Lens Extraction in Severe Myopia

To compare the efficacy and safety between clear lens extraction [CLE] and cataract extraction [CE] in severe myopia, we reviewed the clinical data of 48 eyes of 36 patients who had clear lens extraction to treat axial myopia of 27 mmaxial length or more and 47 eyes of 36 patients who had cataract surgery to treat cataract in axial myopia of 27 mmaxial length or more. Mean follow-up was 19.8 months in CLE and 14.4 months in CE. Visual acuity, complication, and refractive stability were assessed. Best corrected visual acuity of 20/40 was achieved in 79.2% of eyes in CLE and 66.0% in CE. Thirty two eyes [66.7%] were within 1.00D of targeted refractive error in CLE and 20 eyes [42.6%] in CE, postoperatively.The incidence of retinal detachment was 2.1% [1 eye] in CLE and 2.1% [1 eye] in CE. Visual acuity and refractive outcomes with CLE are more favorable than that of CE in severe myopia. The above results support the rationale of the clear lens extraction to treat severe myopia.
