Differentiation and proliferation of ameloblastic carcinoma : immunohistochemical study using Ki-67, CK-LM and CK-HM antibody

Abstract

The ameloblastic carcinoma is an extremely rare, aggressive odontogenic neoplasm of the jaws. It is described as an ameloblastoma in which there is histologic evidence of malignancy in the primary or recurrent tumors, regardless of metastasis. The purpose of this study was to investigate the relationship between proliferation and differentiation of epithelial cell and aggressiveness and recurrence of tumor. The investigation was performed using Ki-67, CK-LM and CK-HM antibody to reveal the proliferation and differentiation of ameloblastic carcinoma. The obtained results were as follows: 1. In H and E stainning, the epithelial cells shows cellular atypism, such as pleomorphism, increased abnormal mitosis, reverse polarity. 2. The positive reaction rates of cell proliferation marker(Ki-67) were higher in ameloblastic carcinoma than in ameloblastoma. In ameloblastic carcinoma, the LI was higher in the peripheral area(18.97+/-2.19) than in the central area(7.19+/-1.13). In ameloblastoma, the LI was higher also in the peripheral area(3.93+/-1.75) than in the central area(1.68+/-0.86). 3. The positive expression of CK-HM were extremely higher in peripheral cells of ameloblastic carcinoma. There was not significant difference between CK-LM and CK-HM in central area. From the above results, we presumed that the invasiveness and recurrence of ameloblastic carcinoma is contributed by its high proliferation and poor differentiation.
