Ergonovine Maleate Provocative Test in a Patient with Variant Angina: A Case Report

Abstract

We present a case of ergonovine maleate provocative test in a patient with variant angina, 57-year old male, who suffered from recurrent unprovoked chest pain at rest in the early morning. He showed normal electrocardiographic findings at symptom free state, but elevation of ST segment was noted in the precordial lead(V5) and hypotension. Coronary angiogram revealed complete occlusion of the first diagonal branch due to focal spasm. The spasm was reversed by immediate sublingual nitroglycerin with restoration of ST change and blood pressure. There was no other specific complication. Thereafter, he has been doing well on long-acting nitrate and calcium antagonist. We conclude that ergonovine maleate provocative test is useful in diagnosing Prin-zmetal's variant angina, but further study is recommended to delineate the optimal protocol for Korean patients.
