Five Minutes Urease Test as Endoscopy Room Test for Helicobacter pylori

Abstract

Background/Airns: We examined the diagnostic efficiency of ultrarapid urease test (1 minute record and 5 minutes record). From our previous study, we already demonstrated that ultrarapid urease test was a reliable diagnostic tool and the 5 minutes record was more sensitive than the 1 minute record. In this study, we check the possibility that the 5 minutes urease test can replace the CLO test used as the standard endoscopy room test.
METHODS
One hundred three patients (58 nonulcer dyspepsias; 21 gastric ulcers; 17 duodenal ulcers; 7 gastric cancers) were included in our study and gastroduodenoscopic antral biopsies were carried out. To detect Helicobacter pylori (H. pylori), three diagnostic tests were performed. 1) In tbe histology, 2 or 3 antral biopsies were treated with hematoxyline & erosin, Giemsa stain. 2) In the CLO test, we used one antral biopsy and recordled the result at 20 minutes & 24 hours later. 3) In five minutes urease test, one antral tissue was placed in ultrarapid urease test kit (capped test tube with 5 ml of 10 % urea & two drops of 1 % phenol red) and incubated at 37C for 5 minutes. If the color was chaged to red, we regarded it as the positive result. When the results of histology and CLO test were positive concunently, we considered as the patient is infected by H. pylori.
RESULTS
By this criteria of positivity, the 5 minutes urease test produced a sensitivity of 98.2 % and a specificity of 100%.
CONCLUSIONS
In comparision with the histology and CLO test, tbe 5 minutes urease test is quicker and cheaper diagnostic test to detect H. pylori. It also provide the almost same accuracy as that of CLO test.
