Abstract

Coronary restenosis in bare metal stents (BMS) is a life-threatening complication in diseases such as unstable angina, myocardial infarction, and congestive heart failure. In-stent restenosis (ISR) in BMS is related to excessive neointimal hyperplasia, late regression, and other thrombogenic factors. Because stented lesions undergo re-endothelialization, very late-ISR (VL-ISR) is an extremely rare complication after BMS implantation. We experienced a case of VL-ISR 7 years after BMS implantation.

Figure

  • Fig. 1 Coronary angiographic findings in July 2002. (A) Critical stenosis in the midsegment of the left anterior descending artery is shown. (B) After the bare metal stent is implanted in the target lesion (arrow), good patent flow is shown at the stented lesion.

  • Fig. 2 Coronary angiography in October 2007. (A) Implanted bare metal stent (arrow) is shown on simple radiography. (B) No coronary artery stenotic lesion is noted.

  • Fig. 3 Coronary angiography in March 2009. (A) Total occlusion (arrow) without distal flow is shown in the implanted bare metal stent lesion. (B) Collateral flow from right coronary artery is shown. (C) After a drug-eluting stent is inserted in the target lesion, good patent flow is shown in the stented lesion.
