We invented a new method to improve the patency and pregnancy rate in microscopical vasovasostomy using the intravasal silicone tube permanently. The silicone tube has a longitudinal slit at one side and is 5mm in length and 0.8mm for an outer diameter and 0.5mm for an inner diameter. A 8-0 nylon suture with needles at both ends is fixed at the middle portion of the silicone tube. Serum for sperm antibody and semen are obtained pre-operation and 3weeks postoperatively. Thirty-one patients have received this operation, 29 of them received vasectomy previously and 2 of them failed in the 1-layer nonsplinted vasovasostomy. Sperm counts were normal numerical state in all cases. Sperm motility was below the normal limit in eighteen patients, but was over 60% with forward progression in thirteen patients. Sperm morphology was more than 60% of oval form in all cases. Sperm antibody was positive in 27 cases. Pregnancy rate was 26%. This method brings normal patency in microsurgical vasovasostomy because the silicone tube prevent obstruction of the anastomosed site of the vas permanently. 100% anatomical patency rate was acquired.
