Treatment of varicocele is required if it is thought to contribute to infertility or is painful or so large as to disturb the patient. The most useful procedure is high ligation of the internal spermatic veins by surgery or, recently, percutaneous occlusion of the veins using balloon catheter or sclerosing agent by radiologic means. These operations have sole aim occluding of a nonfunctioning or refluxing venous pathway, but no tentative restoration of a hemodynamically correct venous drainage of the testis is provided. Herein, we report four varicocele repairs in four patients by microvascular anastomosis between the internal spermatic vein and the inferior epigastric vein for efficient venous drainage with good results.
