Report of Six Cases of Aneurysm of the Posterior Cerebral Artery

We describe six cases of aneurysm of the posterior cerebral artery ; two were small, three were large, and the other was giant, while all were saccular. In two cases, they arose from the P1 segment ; in two, from the P2 segment ; and in two, from the P3. All patients underwent surgery. All P1 aneurysms involved the pterional approach, while for P2 and P3 aneurysms, the subtemporal approach was used. Two small P1 aneurysms were treated by clipping the aneurysmal neck ; two large P2 and one large P3, by trapping ; and one giant P3, by clipping the afferent artery. In five patients, the outcome was either excellent or good, but in one, it was poor ; in this case, the initial neurological grade was poor, though there had been no complications during surgery. We discuss the operative approaches and procedures in relation to the anatomy of posterior cerebral artery aneurysms and also discuss the safety aspect of trapping and/or afferent artery clipping of aneurysm of the posterior cerebral artery.
