Urodynamic Studies after Radical Hysterectomy

Abstract

An extended radical Wertheim hysterectomy is commonly used in the primary treatment of carcinoma of cervix. Postoperative voiding dysfunction, characterized by a hypotonic or atonic detrusor, commonly occur. It is not generally known that hypertonic bladder dysfunction can also occur as postoperative complication. Herein we performed urodynamic studies in 30 patients who underwent radical hysterectomy from August, 1987 through July, 1988. The following results were obtained. 1. Cystometry from two to three weeks postoperatively showed a low compliance hypertonic pressure curve in 17 of 20 patients. Repeat cystometry from four to twelve weeks postoperatively showed diminished hypertonic patterns. 2. Urethral pressure profiles showed a significant decrease of maximum urethral closure pressure in 6 of 10 patients. 3. Uroflowmetry showed intermittent low flow curve postoperatively in 14 of 20 patients.
