Usefulness of MR Imaging in the Assessment of Perianal Fistulous Disease

To evaluate the usefulness of magnetic resonance imaging (MRI) in the preoperative evaluation of perianal fistulous disease. Surgically confirmed 22 cases (anal fistulas, 11 cases and anorectal abscesses, 11 cases) were examined by using MRI. Using 1.5T MR system with a pelvic array coil, T1-weighted spin echo, T2-weighted fast spin echo, and Gd-enhanced T1-weighted images were obtained. We evaluate the agreeement rate between MR imaging finding and operative findings in classification of anal fistula, classification and extension of anorectal abscess, and detection of internal opening. Agreement rate in the classification of fistula was nine of 11 cases (90%) in MRI. Agreement rate in the classification and extension of abscess was 11 cases (100%) in MRI. The internal opening was detected in 12 of 21 cases with operative findings. The internal opening was detected in 10 of 12 (83%) with MRI. MRI can be a useful diagnostic method in classification and extension of anorectal abscess.
