A Case of Subphrenic Abscess Caused by Dropped Gallstones during Laparoscopic Cholecystectomy

Abstract

Laparoscopic cholecystectomy has been a preferred treatment method for patients with symptomatic gallstones. However, it is associated with a risk of injury to the bile duct or gallbladder. We recently experienced a case of subphrenic abscess due to gallstone dropped during laparoscopic cholecystectomy in a 60-year-old male. The patient who had received laparoscopic cholecystectomy 6 months ago admitted with right upper quadrant pain for 7 days. Abdominal CT scan showed a abscess in subphrenic area and sono-guided aspiration confirmed pus within the lesion. After the aspiration abdominal pain was subsided, but it recurred two weeks later and the lesion was enlarged on follow-up abdominal sonography. Exploratory laparotomy was performed and it revealed multiple gallstones within the abscess.
