Clinkal Observation on 28 Cases of Simplified Dacryocystorhinostomies

Abstract

From January, 1973. to June, 1976 the Auther Performed simplified dacryocystorhinostomies on 28 cases with tearing and mucopurulent material discharge from the lacrimal sac. The auther's surgical technic was a modified Iliff technic and the addition of the polyethylene tube fistulization procedure for complicated case of lacrimal sac are introduced. The results of observations are sumerized: 1) Among 28 cases, 15 of the subjects r were male and 13 of the subjects were female. The ages of the patients ranged as follows: The youngest was 7 years of age and the oldest was 59 years of age. 2) The success rate of this method was 89% and Author's polyethylene tube fistulization procedure in selected case gave a higher success rate. 3) When the occulusion of the canaliculi, lower and upepr punta was noted, the polyethylene tube fistulization procedure was added. 4) Some common complicacion, mild wound bleeding and subconjuntival hemmorrhage etc, were observed. This complication could be easily controlled by adequate treatment. 5) The explanation for the obstruction was apparent in fewer than half of the patients. We can assume that the 2 children had a congenital membrane at the lower end of the tract. All of these tracts had had repeated probings. 3 patients had injuries with breaks in the bony lacrimal canal. In 2 patients the obstruction had been caused by chronic sinusitis. 6) Among the 25 successful cases, the symtoffis were relieved immediately in 24 cases, but tearing and mucopurulent material persisted in 1 case. So a probe was passed into the nose to reestablish patency of the tract. In Conclusion, the operation is simple to do and within the range of any ophthalmic surgeon. A simplified dacryocystorhinostomy is the best treatment of tearing and mucopurulent material drainage from the nasolacrimal duct.
